- Workforce events involving students, staff and or faculty, should be reported through OARS (requires UWNetID)
- Patient and visitor events should be reported using the Patient Event Form in RequestManager. You will be prompted to enter your UW email address and taken to the standard UWNetID login screen. Once you are logged in, choose the New Request button, and Patient Event Report as the Request Type.
