If you are a dentist, physician, or other health care professional who wishes to refer a patient to the UW Dentistry Faculty Practice, please use the HIPAA compliant online form at the link below and provide as much information as you can. We will also provide you with periodic updates regarding the patient’s treatment.
For Patients
If you are a patient whose dentist, physician, or other health care professional has advised you to seek oral health care at the UW Dentistry Faculty Practice, you have two options for starting the process for being seen here. You may contact the referring provider and request that he/she access the link above and complete the referral form OR you may contact us directly at 206-685-8258 to schedule an initial appointment. Please have the name and contact information of the referring provider available if you call.
