Oral Radiology Referrals

To refer a patient for imaging at our radiology clinic, you may use your own letterhead, or use our Oral Radiology Referral Form (PDF).  We can provide same day imaging during normal business hours. Call 206.543.5006 to make an appointment. All imaging includes an interpretation report.

CBCT and 2D Image Reading Service

To refer a CBCT volume for an interpretation, please use our Online Submission Form.  For HIPAA compliance we request that you use our secure server for uploading cases.  After submitting the form, an invitation should come in an email from LifeImage.  Please allow up to 48 hours for the invitation.  Currently, we are having to manually invite users for case uploads, but we are working on a more automated attachment process. You may refer a patient for imaging and interpretation of suspected pathology or simply for imaging when you require a CBCT for guidance of a planned procedure.

If mailing a CD or DVD is preferred, use our CBCT Interpretation Request Form (PDF) and send the disk and request form to the address listed on the form.

*Note to referring provider:
Payment for the Oral Radiology Service is the sole responsibility of the referring provider and is billed at the end of the calendar month.  Please allow 3-5 days for all imaging reports.
