Your account number is located in the upper right of your billing statement. If you do not have a statement, or if you are unsure of your account number, please call UWSOD Patient Billing Services, Monday through Thursday, between 9:00 a.m. and 3:00 p.m., at 206-543-5297. Both patient and payment information will be kept secure and confidential.
The UW School of Dentistry is committed to compliance with all state and federal laws relating to patient or third party overpayments for services provided in the school’s dental clinics. The school’s policy is to return all monies due to patients, third-party payers and any other party having a patient account with the School of Dentistry within 60 days from the date an overpayment or other credit balance is identified.
