Privacy, Confidentiality, and Information Security Agreement

Digital communications have increased the ease with which information can be accessed, and this has necessitated changes in our efforts to protect patient, co-worker and student privacy and confidentiality.

Why This is Important

The Agreement is essential to ensure our compliance with federal and state regulations. We must maintain data integrity and protect patient information, student information, workforce records and other restricted and confidential information. Because it is critical that we each understand and agree to adhere to these conditions, review and signature of the Agreement is required at regular intervals for all workforce members. Workforce members include faculty, employees, students, trainees, volunteers and other persons who perform work for UW School of Dentistry regardless of whether or not that member is paid by UW School of Dentistry.

Workforce Responsibilities

As a user of UW and UW School of Dentistry computing resources and data, you are required to review and sign this Agreement. The Agreement confirms your responsibility to comply with UW and UW School of Dentistry policies; to protect your accounts and passwords; to not change computer configuration without official approval; to keep patient and student information confidential and to use UW resources for authorized duties only.

Computer Configuration

This Agreement references computer configuration. “Computer Configuration” refers to the combination of hardware, software, operating system, and the settings used to set up each of these items. These settings apply to all users of the system and are not changeable except by someone with elevated privileges. Examples of computer configuration are operating system settings, firewall settings, system time, and installed applications that require a change to the system set-up. Alteration of these could introduce security risks or damage/disable a computer if not done properly.
