How do I become a patient?

The University of Washington Northwest Center for Oral and Facial Surgery phone hours are from 7:00 a.m. to 11:00 a.m. and 11:30 a.m. to 3:30 p.m., Monday through Friday. For appointments, patients should call 206-543-5860 and select option #1. Your provider should submit a Northwest Center for Oral and Facial Surgery Referral Form* before your appointment.

*This form uses REDCap, a HIPAA compliant platform.

Fees and Insurance

Many oral and maxillofacial surgery procedures are covered by medical insurance.  In order to facilitate best coverage, please bring both your medical and dental insurance cards to your appointment.

Clinic Location


The Northwest Center for Oral and Facial Surgery is located at 6222 NE 74th Street, Seattle, the Washington Dental Services Building at Magnuson Park. Parking is free. Free Wi-Fi access is available to patients and visitors.

Emergency Care

If there is an emergency after being treated at the Northwest Center for Oral and Facial Surgery, the patient should page the “oral and maxillofacial surgery resident on call” at our affiliated Harborview Medical Center by calling 206-744-3000 after hours, weekends or holidays. Please be sure to identify yourself as a patient at the Northwest Center for Oral and Facial Surgery. During regular business hours (7:00 a.m. to 3:30 p.m.), post-operative questions and problems should be addressed by calling the Clinic 206-543-5860.  If you experience a life-threatening emergency, dial 911.

Oral and maxillofacial surgeons are licensed to administer office-based anesthesia, including local anesthesia, conscious sedation, intravenous sedation, and general anesthesia, providing a range of options for the comfort and safety of our patients.

Dr. Dodson

Dr. Thomas B. Dodson, Oral Surgeon, Chair

Dr. Thomas Dodson, DMD, MPH, is Professor and Chair of the Department of Oral and Maxillofacial Surgery. Formerly Professor of Oral and Maxillofacial Surgery at Massachusetts General Hospital and Harvard School of Dental Medicine, Dr. Dodson is a Diplomate of the American Board of Oral and Maxillofacial Surgery. Dr. Dodson is Chief of Service at the University of Washington Medical Center and practices the full scope of oral and maxillofacial surgery.


Dr. Beirne

Dr. O. Ross Beirne, Oral Surgeon

Dr. Ross Beirne, DMD, PhD, is Professor of Oral and Maxillofacial Surgery and Associate Adjunct Professor at the University of California, San Francisco. Dr. Beirne is a Diplomate of the American Board of Oral and Maxillofacial Surgery and the National Dental Board of Anesthesiology.


Dr. Bloomquist

Dr. Dale S. Bloomquist, Oral Surgeon

Dr. Bloomquist, who is an Associate Professor Emeritus in the Department of Oral and Maxillofacial Surgery, is a diplomate of the American Board of Oral and Maxillofacial Surgery. He is an attending surgeon at the UW Medical Center, Harborview Medical Center, Seattle Children’s Hospital, and Swedish Hospital and Medical Center. He holds dental licenses in Washington and Idaho, and is board-certified by the American Board of Oral and Maxillofacial Surgery. Dr. Bloomquist has limited his practice to orthognathic surgery for the last 20 years.


Dr. Andrea Burke

Dr. Andrea Burke, Oral Surgeon

Andrea Burke, DMD, MD, is a Clinical Assistant Professor in the Department of Oral and Maxillofacial Surgery. Dr. Burke joined the Oral and Maxillofacial Surgery team at the University of Washington after completing the Dental Clinical Research Fellowship Program at the National Institute of Dental and Craniofacial Research, National Institutes of Health, in Bethesda, MD. She trained in Oral and Maxillofacial Surgery at the Massachusetts General Hospital in Boston, MA, and obtained her D.M.D. and M.D. degrees from Harvard University. Dr. Burke is a Diplomate of the American Board of Oral and Maxillofacial Surgery. She practices the full scope of Oral and Maxillofacial Surgery, with a special interest in skeletal diseases.


Dr. Zahid Lalani

Dr. Zahid Lalani, Oral Surgeon

Zahid Lalani, DDS, PhD, MBA, is a Clinical Assistant Professor of Oral and Maxillofacial Surgery. Dr. Lalani is a Diplomate of the American Board of Oral and Maxillofacial Surgery and National Dental Board of Anesthesiology. He is a Fellow of the Royal College of Surgeons of England and Ireland. Dr. Lalani completed his training in oral and maxillofacial surgery at the University of Texas Health Science Center in Houston and trained in India and the UK prior to that. He completed his doctoral studies in Molecular Biology with an emphasis on wound healing as part of a joint residency/PhD program. Prior to joining the oral and maxillofacial surgery team at the University of Washington, Dr Lalani was in private practice in Houston for 19 years. Dr. Lalani practices the full scope of oral and maxillofacial surgery.


Dr. Melanie Lang

Dr. Melanie S. Lang, Oral Surgeon

Melanie Lang, DDS, MD, is a Clinical Assistant Professor of Oral and Maxillofacial Surgery. Dr. Lang is a Diplomate of the American Board of Oral and Maxillofacial Surgery and the National Dental Board of Anesthesiology. Dr. Lang practices at both the University of Washington and Harborview Medical Center. Prior to joining the Oral and Maxillofacial Surgery team at the University of Washington, Dr. Lang was in private practice for fourteen years. Dr. Lang practices the full scope of Oral and Maxillofacial Surgery with a special interest and fellowship training in Facial Cosmetic Surgery.
